This form appears automatically while you are in demonstration mode whenever you open the program.
It also appears when you click on the "Show License Status" button on the Main Switchboard. It contains the following data elements:

| The City, State, and Zip of your church (taken from the My Church form) |
| A text box in which to display or enter your User Identification Key. |
| A text box in which will be displayed the number of days remaining for your use. This display is activated or recalculated when you tab out of the User Identification Key text box. |
When you start up the program initially, you must enter the information about your church. An algorithm will convert your church's data elements into a numeric key which is not displayed. You will have 60 days to determine if you like the program. 60 days after the program is used for the first time, you cannot get past this form to use the program unless you are supplied a key by ChurchMouse Development Corp. When you receive the key, it will be based on your church name and address as supplied to ChurchMouse Development; obviously, the data you supply to ChurchMouse and the data entered in this form must be identical.
This key must be entered in the "Enter Key" text box. When you exit the Enter Key box, a calculation will be made to see if the key you just entered agrees with the key calculated internally. If so, the remaining days of use will be set to 9999. If not, you will be locked out of the program if you have no remaining days.
After the key has been entered properly, you will not see this form unless you click on the "Show License Status" button on the Main Switchboard, or unless you change the name/address of the church. If you get a notification that your period has expired, it also displays a button to allow you to access the "My Church" form so that you can change the name/address back to what it had been previously. At that point, you must contact ChurchMouse Development again to obtain a new key.
This license is not transferable, and the program may be used only by the church originally purchasing the license.
